Forms & Documents

Everything you need to register is here. Print and fill in the three forms, then bring them to the school or email them to us. We will confirm your spot and send your Parent App invitation once your deposit is in.

Four steps

How to register

  1. Register

    Complete the registration form and return it with one month's fees by e-transfer. That deposit is applied to your first month.

  2. Tell us about your child

    The All About Me form helps your child's teacher know them before day one.

  3. Immunization record

    Ask your doctor's office to complete the immunization form, or bring your child's yellow card so we can copy it.

  4. Join the Parent App

    Once registration is complete you receive an invitation. Fees, newsletters and closure notices all come through the app.

Return completed forms in person at 85 Sandalwood Parkway East, or scan and email them to .info@buildingblocksnursery.ca.
